Adapters resolve a spec per key prefix, so the enforced limit is not
always the one the caller configured: an operator who declares
limits.login on the adapter overrides the value seeded from
rateLimit.login, and a key with no matching prefix falls to
defaultLimit. A caller emitting RFC 9239 RateLimit-* headers has to
report what was enforced, not what it asked for, or the headers
advertise a limit no request is measured against.
Optional so pre-existing custom adapters keep compiling; callers fall
back to their own configured value when it is absent.
